Salary Range : $70,000 - $85,000 Commission
For nearly two decades, Health Link Home Health and Hospice has been dedicated to providing exceptional healthcare services to patients throughout the San Francisco Bay Area, all in the comfort of their homes. As we continue to expand, we're seeking a dynamic Community Liaison to help advocate for our home health services across the Bay Area, including the Peninsula and surrounding regions.
About the Role :
As a Community Liaison at Health Link Home Health and Hospice, you will play a vital role in fostering relationships and building trust within the community. Your primary focus will be leveraging Health Link's strong local presence to promote our home health programs and services.
In home health, trust is everything. You'll be the face of our organization, ensuring that doctors, nurses, social workers, and other healthcare professionals trust us with their patients. Your responsibility is to champion the quality care provided by our clinical team and increase the number of patients who benefit from our services.
You'll collaborate closely with our Community Relations Manager and CEO to implement a comprehensive sales and marketing strategy that aligns with our goals. A solid understanding of sales and marketing will be key, and while prior experience in home health or hospice is beneficial, it's not required. What's important is your ability to learn quickly and effectively apply new information to drive success.

Key Responsibilities :
Sales & Marketing Strategy : Implement and execute a comprehensive sales and marketing plan to promote Health Link's Home Health services across the region.
Relationship Building : Cultivate and maintain strong relationships with key community partners including hospital case management and social work teams, physicians, senior living communities, skilled nursing facilities, and more.
In-Person Outreach : Conduct in-person visits, attend meetings, and deliver presentations on our services to potential referral sources. Follow-up consistently to maintain and strengthen relationships.
Coordination & Communication : Work closely with the intake team to ensure smooth referrals and serve as the liaison between external partners and internal staff. This will involve regular messaging and coordination calls.
Community Representation : Represent Health Link at relevant community events, conferences, and meetings to build awareness, forge new connections, and strengthen our local network.
Budget & Resource Management : Manage your own budget and track expenses to ensure efficient use of resources.
Referral Development : Identify, target, and develop new referral sources to expand our network and increase service utilization.
Responsiveness & Organization : Maintain excellent communication with your team and community partners, and manage your calendar and CRM system with precision. Always be responsive, except during planned time off.
Qualifications :
- Bachelor's degree and experience in sales, account management, or territory growth.
- Proven ability to build and manage relationships in a sales-driven environment.
- Strong communication and organizational skills.
- Ability to adapt quickly and learn about the home health and hospice industry.
