Position Summary :

As a Nurse Practitioner (NP) in our Palliative Care Program , you will provide expert care to patients facing acute and chronic conditions, working closely with physicians and interdisciplinary care teams. You will be instrumental in managing end-of-life care, symptom management, and fostering comfort and dignity for patients and their families. This role offers a unique opportunity to work in a compassionate, patient-focused environment, where your expertise in palliative care will have a direct impact on improving patients' quality of life.

You will report to the Director of Nursing Support Services and collaborate with a dedicated team of healthcare professionals. The ideal candidate will bring strong clinical skills, an ability to assess and manage complex health conditions, and a deep sense of empathy to address the physical, emotional, and psychological needs of patients.

Key Responsibilities :

• Manage Day-to-Day Operations : Oversee the day-to-day operations of the Palliative Care Program, ensuring quality care is delivered to all patients.
• Coordinate Patient Services : Lead the coordination of direct and indirect patient services, working with a multidisciplinary team to ensure seamless care delivery.
• Staff Orientation & Development : Assist in the orientation of new personnel and promote ongoing development of clinical staff to ensure excellence in patient care.
• Palliative Care Delivery : Provide comprehensive palliative care through health assessments, diagnosing conditions, developing care plans, prescribing medications (within licensure scope), and coordinating care with the patient's family and other healthcare providers.
• Collaborative Care Planning : Work with physicians and interdisciplinary team members to create individualized care plans tailored to the patient's needs.
• Symptom & Pain Management : Use a patient-centered approach to discuss end-of-life issues, symptom management, and provide comfort to patients and families during difficult times.
• Referral and Consultation : As needed, make referrals or consult with other specialists to ensure holistic, multidisciplinary care is provided.
• Community-Based Care : Utilize community resources to design and direct care that meets the needs of patients and their families in their home environment.
Qualifications :

Active California RN License and Certified Nurse Practitioner status.
