What You'll Do:
- Conduct thorough patient admissions, including assessments and care plan development.
- Provide education to patients and families on care plans, medications, and home health services.
- Collaborate with physicians and clinical teams to ensure smooth care transitions.
- Document all admission details accurately in the electronic medical record.
- Ensure compliance with regulations and company standards.
What We're Looking For:
- Current California RN License.
- 1 Year Home Health Experience.
- Strong communication, organizational, and time management skills.
- Tech-savvy and comfortable using electronic medical records.
- Compassionate and committed to delivering exceptional patient care.
Schedule / Work Location:
- Flexible weekends - Contra Costa County
Experience:
- Home Health: 1 year (Preferred)
- BLS Certification (Required)
- RN License (Required)
- Driver License (Required)
Pay:
- Per Visit: $150-$230
- Hourly: $65-$82
