Clinic Director Licensed Occupational Therapist (OT) - Care Coordination

We are seeking a Clinic Director who is a licensed Occupational Therapist (OT) to lead our innovative care coordination program for seniors. This role allows you to practice at the top of your license while engaging in care coordination duties. Responsibilities include overseeing care delivery, conducting wellness visits, leading operational initiatives, and providing occupational therapy services.
