We provide blood pressure monitoring as well as heart failure monitoring and chronic disease monitoring in Nursing Homes and Assisted Living Facilities. We combine high touch, high tech care for preventing developing and worsening of cardiovascular and chronic conditions.
We collaborate with primary care providers, other cardiologists and various team members to coordinate care.
We are seeking a highly organized and experienced Nurse Practitioner to provide in person rounding at Skilled Nursing Facilities. As the Nurse Practitioner, you will play a crucial role in ensuring the smooth and efficient functioning of the in person clinical preventive assessments and the remote monitoring service, providing exceptional patient care. This position requires strong organization skills, attention to detail, and a deep understanding of both medical and administrative procedures within the medical field.
