The Health Navigator Foundation supports patients facing the need for complex life saving surgeries including heart and liver transplant and LVAD implantation. We provide the essential resources of housing, food, parking and gas during the critical transition from hospital discharge to recovery. We work as a partner organization with California Pacific Medical Center's transplant prorgam to provide support to their patients.

Role Description
This is a temporary, hybrid role for an Interim Program Director at Health Navigator Foundation in San Francisco, CA, with flexibility for remote work. Coverage is need from February 2025 through mid October 2025. The Interim Program Director's will:
● Provide navigation services to patients who have been referred to the Foundation
as well as practical and resource support for patients and caregivers
● Manage resource allocation to Foundation participants including housing, meal and
transportation services, and assist with locating vendors for the Foundation
● Work in collaboration with the team of physicians, nurses and social workers in the transplant department
● Supervise management of programmatic budgets to ensure effective and compliant
use of grant and contract funds
● Conduct research including gathering and scoring quantitative data and
administering qualitative interviews
● Oversee and maintain the peer mentor group, match patients with peer mentors
and follow-up on peer connections
● Facilitate monthly virtual heart transplant and LVAD patient support group and caregiver
support group
● Assist patients and families with understanding and accessing resources available to
them including community mental health services, federal and state work leave,
insurance and financial/nutrition assistance programs
● Direct, lead, and manage the development of all grant writing and fundraising for
the program
This is a hybrid schedule consisting of 3-4 days working from home and 1-2 days working onsite at the CPMC clinic on Van Ness Ave.
Qualifications
- Program Management, Strategic Planning, and Budget Management skills
- Experience in overseeing program operations and implementing initiatives
- Experience in group therapy preferred
- Strong Leadership and Communication abilities
- Ability to work in a hybrid setting with some remote work
- Experience in the healthcare or non-profit sector
- MSW required, LCSW preferred
