Health Plus Indiana is a non profit agency providing services and education to people living with HIV and the community. Our mission is to enhance the lives of those infected with and affected by HIV. To do this we educate and test the community for HIV, HEP C, and STIs. We provide transportation, housing, case management, and clinical services. Our services are taken to the community, meeting people where they are, and leading them to self sufficiency.
Our Client Services Department is looking to hire a Case Manager for the Main office in South Bend. Duties for this position include managing an 80 person caseload, assessing the needs of Client's and connecting them with needed services. We offer a wide range of services. The Main Office is currently open Monday-Thursday. We will be operating on a hybrid schedule, with Monday-Thursday hours from 8:30am-4:30pm being in office and Friday hours from 8:30am-3:00pm being remote. We work one late night a month on Tuesday's until 6:30pm. Some local travel is required. Associates degree in applicable field is required. Case Management experience preferred.
