We have 11 clinics located throughout the River Region, 2 mobile clinics, and 4 school-based clinics in partnership with Montgomery Public Schools
HSI is looking for a Case Manager to join our team. The Case Manager assesses patient treatment needs through developing, monitoring, and evaluating treatment plans and progress.
This position typically works a standard schedule of 7:30am to 4:30pm Monday through Friday, however, schedule adjustments may be necessary based on need.
Job Duties:
- Assist clients in accessing available health care, social services, and related systems in order that outcomes which contribute to physical and emotional health wellness and the minimizing of economics and psychological distress.
- Conducts welcome calls for uninsured patients to provide information regarding service and required documentation.
- Provides support and resources for patients thru collaboration with the clinical team.
- Contributes toward and promotes the coordination of relevant health and social care services in order to optimize patient benefits.
- Conducts intake interviews, performs psycho/social assessment to determine needs and development of case management plan.
- Acts as a patient advocate and presents their needs and preferences for care when supporting the procurement of services.
- Coordinates treatment programs by setting schedules and routines and follows up on services being provided.
- Monitors cases by verifying the patients' attendance with appointments and evaluating response to treatment.
- Ability to prioritize and apply available resources in the most efficient and clinically effective manner to ensure high quality patient care.
- Assist patients in applying for social program benefits where applicable.
- Charts health data information as appropriate in medical records.
- Keeps current with services and support networks available in the service area and be proficient in applying them to the problems of health and human services delivery systems.
Education and Experience:
- Bachelor’s Degree in Social Work with two (2) years; RN with one (1) year; or LPN with three (3) years case management experience.
- Possess some knowledge of medical terminology, interpreting orders and developing a plan of managed care in accordance to physician request.
