What You Would Be Doing
As a Clinical Care Specialist with Adult Outpatient services, you will provide office-based services to help our clients access treatment and resources and take their next steps to wellness. You will use your skills to assist clients in managing and overcoming their symptoms and difficulties to improve day-to-day functioning, support other members of the treatment team, and collaborate with professionals within and outside our agency to assure that clients receive the very best care available.
Specifically, the Clinical Care Specialist works to:
- Teach coping, symptom management, and daily living skills;
- Identify stressors and precursors to crises in order to aid in development of coping plans;
- Identify coping mechanisms and natural supports that may be available;
- Provide education regarding mental health;
- Enhance problem-solving skills;
- Link clients with community resources;
- Help clients to make and keep scheduled appointments with medical providers;
- Provide and/or arrange transportation for scheduled appointments;
- Keep accurate, complete and up-to-date records with 100% compliance with company standards;
- Maintain competency and proficiency with the agency’s Electronic Health Record;
- Perform other job-related duties assigned by the director or his/her designee.
Physical requirements include the following:
- Frequently stays stationary for prolonged periods of time.
- Frequently moves within and between facilities.
- Frequently exposed to stressful or emergent situations.
- Frequently operates a computer and other office equipment to record health data.
- Constantly communicates with clients and others.
- Occasionally moves equipment and/or materials up to 20 pounds.
Qualifications
What We’re Looking For—The Must-Haves
- Bachelor’s degree in social work, psychology, criminal justice or a closely related field
- Experience in case management/community systems navigation
- Experience working with persons diagnosed with severe & persistent mental illness and substance use disorders
- Exceptional flexibility, time management and documentation skills
- Must be a self-starter and self-motivator
- Computer proficiency
- Colorado driver’s license, a good driving record, and access to transportation during work hours

- Experience successfully working in multi-disciplinary teams
- Community systems navigation background
- Spanish language skills
