Were looking for a Clinical Care Specialist to join our Rural Services team in our Walsenburg clinic.
What Youd Be Doing
As a Clinical Care Specialist with Rural Services, you would play a central role in helping individuals in the region of Huerfano and Las Animas counties access services and resources and take the next steps to wellness by providing foundational behavioral health counseling and skills development. You would use your professional case management skills to assist clients in developing daily living and other skills central to maintaining stability and overall wellness. You would use your skills in motivational interviewing and cognitive behavioral skills training to help motivate clients to achieve their treatment goals. You would interface with other members of the treatment team and collaborate with professionals within and outside our agency to assure that individuals in the Rural Service region of Huerfano and Las Animas counties receive the very best support and care available to manage their mental and physical health.
Specifically, the Clinical Care Specialist:
- Makes appropriate referrals to address mental health and substance use needs;
- May conduct client home visits;
- Helps ensure clients attend their scheduled appointments in the community and at Health Solutions;
- Provides behavioral health counseling, group treatment, and resource based case management
- Collaborates with other treatment providers to ensure a team based approach to treatment, when applicable
- Identifies coping mechanisms and natural supports that may be available;
- Teaches and enhances client coping, symptom management, problem-solving, independent living, and socialization skills;
- Develops, implements, and reviews treatment plans with clients;
- Maintains competency and proficiency with our electronic health record system;
- Performs other job-related duties assigned by the director or his/her designee.
Physical requirements include the following:
- Requires sitting for prolonged periods of time
- Occasionally requires driving
- Occasionally lifts and carries items up to 20 pounds
- Requires working under stress in emergency situations
- Requires normal range of hearing and eyesight to record data electronically and communicate with clients
- Occasional exposure to communicable diseases and bodily fluids
- Requires occasional driving in adverse weather conditions
- Requires frequent use of hands and fingers to operate a computer keyboard and mouse

What Were Looking ForThe Must-Haves
- Bachelors degree in social work, psychology, or a closely related field
- Computer proficiency
