We’re Hiring a School-Based Behavioral Health Professional to join Health Solutions Rural Department, working within the school setting at our Trinidad School District Number 1 and Huerfano School District Re-1 (Trinidad & Walsenburg) in Las Animas and Huerfano Counties.

As a Rural School-Based provider with our rural department, you would be a vital member of a team that works within the students’ familiar school environments to promote mental and emotional health and well-being. Your collaborative work with school staff and other community resources would further enhance the overall wellbeing of the students, their families, and our community both now and in the future.

Specifically, the School-Based Provider will:
- Travel between Las Animas County and Huerfano County providing services to students from Elementary Schools, Middle Schools and the High Schools
- Assist with and support new client processes including enrollment and assessment
- Provide individual, family and/or group counseling services;
- Provide case management services and collaborate with resources in the community;
- Assist with and support link to crisis services and crisis evaluations, as needed;
- Keep accurate, complete and up-to-date records with 100% compliance with Center standards;
- Maintain competency and proficiency with Health Solution’s Electronic Health Record;
- Provide consultation and education services;
- Work in collaboration with the school based wellness center staff;
- Perform other job-related duties as assigned by a Program Director or Supervisor.

Qualifications
What We’re Looking For—The Must-Haves
- Graduate or Bachelor’s degree in social work, psychology, or a closely related field
- Demonstrated computer proficiency and experience with electronic health records
- Colorado driver’s license and access to transportation during work hours
- At least one year of related experience with children/adolescents
What We would Like to See in You—The Nice-to-Haves
- At least one year of mental healthcare experience treating children/adolescents.
- Spanish language skills
