The Mental Health Therapist provides culturally competent, trauma-informed social work interventions, including assessments, treatment planning, therapy, crisis intervention, and aftercare coordination. Responsibilities include:
- Conducting psychosocial assessments and developing individualized treatment plans.
- Participating in interdisciplinary team discussions for discharge planning.
- Building rapport with patients and providing crisis stabilization and supportive therapy.
- Maintaining thorough documentation of services.
- Acting as a liaison with families and community resources.
- Advocating for patients while ensuring confidentiality.
- Two years of post-graduate experience preferred.
- Master's Degree in Social Work or Mental Health Counseling.
- NYS License or Limited Permit License required.
