Job Title: Nurse Navigator - Interventional Radiology & Nuclear Medicine

Summary: The Nurse Navigator will provide clinical support and care coordination for patients undergoing image-guided therapies and theranostic procedures, focusing on improving patient experience and outcomes. This role involves acting as a liaison among providers, patients, and procedural teams to ensure seamless access and timely treatment.

Qualifications

• BSN required; current RN licensure in Georgia
• AHA BLS certification; minimum three years of nursing experience
• Preferred experience in procedural, interventional, oncology, or case management nursing
• Strong clinical assessment, critical thinking, and communication skills
• Proficiency in Electronic Medical Record systems
• Preferred certifications in Radiology Nursing, Oncology, or Case Management
  
  

Key Responsibilities

• Guide patients through pre-procedure education and follow-up
• Ensure timely scheduling and coordination of procedures
• Collaborate with multidisciplinary teams for readiness and authorization
• Track patient progress through various procedures
• Provide ongoing education and support to patients and families
• Monitor missed appointments and coordinate outreach
• Assist in documentation and quality tracking for improvement initiatives
