Job Summary Responsible and accountable for the teaching, assessment, safety and monitoring of chronic and acute hemodialysis patients as well as providing support and education to the patients family. Functions as an intricate member of the multi-disciplinary team. Requirements and
Responsibilities:
- Licensure and Certifications: Employee maintains current licensure and/or required certifications. (If ‘No’ employee is put on leave without pay until in compliance)
- Annual Mandatory
Education:
Employee completes annual Mandatory Education Module, Annual Health Assessment,Age Specific requirements, etc… (If ‘No’ employee is put on leave without pay until in compliance)
- Time and Attendance: Employee complies with all HealthAlliance and departmental policies for time and attendance.
- Professional Appearance: Employee complies with HealthAlliance policy for professional Appearance (Dress Code), including wearing ID Badge.
- Safety – maintains a safe environment for patients, visitors, physicians and employees.
- Applies the nursing process to perform initial and ongoing assessment of patients.
- Recognizes importance of informed consent.
- Follow policies and standards to assess, monitor and recover patients undergoing conscious sedation.
- Follows policies and standards to assess and recover patients during Phase II recovery.
- Evaluates patients learning needs related to procedures and provides education throughout patient stay and upon discharge.
- Accurately documents in the patient record utilizing appropriate documentation in electronic medical record.
- Use proper tools to accurately charge patients.
- Performs any other duties as assigned.
Education:
- Minimum – 2 yr. College degree; preferred – 4 yr. College degree
Experience:
- Minimum – 1 yr. Acute care experience; preferred – 1 yr. Hemodialysis experience
Training/License:
- NYS Registered Professional Nurse; BLS Certifications
Other:
oNephrology nursing certification preferred, med/surg nursing certification preferred.
