HealthBridge Post Acute Rehabilitation, located in Wyoming, MI, is seeking compassionate and dedicated Certified Nursing Assistants (CNAs) to join our team! We offer flexible Part-Time Night Shift and PRN positions for all shifts .
HealthBridge is a dynamic, fast-paced facility that provides top-quality short-term skilled nursing and rehabilitation care. Our modern center is equipped with 65 private rooms, inviting lounge and dining areas, and a state-of-the-art 3,000-square-foot rehabilitation gym. We strive to make the recovery process as comfortable, safe, and effective as possible for our patients.
Position Overview : As a CNA at HealthBridge, you will be an integral part of the team, providing hands-on care to residents and supporting them with their daily needs. You will help create a safe, nurturing, and comfortable environment for residents during their recovery.
Key Responsibilities :
Resident Care : Assist residents with activities of daily living (ADLs), including bathing, grooming, dressing, and oral hygiene.
Incontinence & Toileting Care : Provide assistance with incontinence care, toileting, and maintaining residents' dignity and comfort during these tasks.
Colostomy & Prosthetic Appliance Care : Assist with colostomy care and the use of prosthetic appliances to ensure comfort and functionality.
Transfers & Ambulation : Help transfer residents safely, whether from bed to chair or during mobility exercises, and support them in ambulation (walking) to promote independence.
Range of Motion : Perform range of motion exercises to enhance mobility and prevent muscle stiffness or joint contractures.
Communication : Effectively communicate with residents, families, and the healthcare team to ensure proper care and report any changes in the resident’s condition.
Clinical Documentation : Accurately document all care provided, including vital signs, ADLs, and any changes in condition, maintaining up-to-date and compliant medical records.
Resident Advocacy : Ensure a compassionate, resident-centered care environment by respecting residents’ rights, preferences, and dignity.
Safety & Infection Control : Follow all safety protocols, infection control procedures, and cleanliness standards to maintain a safe and healthy environment for both residents and staff.
Qualifications :
Current CNA certification in Michigan.
Experience in long-term care or rehabilitation settings preferred, but not required.
Ability to work independently and as part of a team.
Compassionate, patient-centered care approach.
Strong communication skills and attention to detail.
Ability to remain calm and composed in high-pressure situations.
