Our modern center is designed to support patients recovering from illness, injury, or surgery with the help of our skilled team of physical, occupational, and speech therapists, nurses, and physicians. Position Overview : As a Charge Nurse at HealthBridge, you’ll be at the forefront of ensuring high-quality patient care during your shift. You will work collaboratively with the healthcare team, supervise nursing staff, and advocate for residents to maintain a safe, nurturing, and efficient care environment.
Key Responsibilities :
Patient Care Coordination :
Assess residents’ healthcare needs and collaborate with the team to develop tailored care plans.
Oversee daily care, medication administration, and ensure residents receive appropriate attention.
Staff Supervision and Leadership :
Lead and mentor the nursing team, offering guidance and professional development.
Delegate tasks effectively, ensuring optimal care delivery.
Medication Management :
Ensure accurate and timely medication administration.
Monitor and maintain proper medication storage and documentation.
Clinical Documentation :
Keep precise and up-to-date medical records for residents.
Ensure documentation is completed in a timely and compliant manner.
Communication & Collaboration :
Foster communication among the healthcare team, residents, families, and other stakeholders.
Collaborate with all disciplines to address residents’ comprehensive needs.
Resident Advocacy :
Champion residents’ rights and preferences in care decisions.
Address concerns from residents or families, working toward effective solutions.
Infection Control & Safety :
Enforce infection control protocols and ensure safety measures are met.
Respond promptly to safety concerns or incidents.
Quality Assurance :
Participate in audits and quality improvement initiatives to meet or exceed healthcare standards.
Contribute to best practices and process enhancements.
Qualifications :
LPN with a valid Michigan nursing license.
Preferred experience in long-term care or nursing home settings.
Leadership, clinical assessment, and problem-solving skills.
Strong communication and interpersonal abilities.
Proficiency in EMR and nursing documentation.
Knowledge of state and federal nursing home regulations.
Compassionate, resident-centered care mindset.
Ability to remain composed in high-pressure situations.
