HealthBridge is a short-term skilled nursing and rehabilitation center that has been designed to help our patients recover after an illness, injury, or surgery. Our team of physical, occupational, and speech therapists, as well as nurses and physicians will work with our patients on an individualized care plan to help them get back to their home. Our goal is to help our patients maximize their independence and mobility and to provide them with the tools they need to help manage their health care. We are seeking a detail-oriented Medical Records (HIC) professional to join our team. This role is crucial for maintaining the integrity and confidentiality of medical records while ensuring compliance with relevant regulations. The ideal candidate will have a strong background in information governance and data management, along with proficiency in various record management systems.

Responsibilities

  • Manage and maintain electronic medical records, ensuring accuracy and compliance with healthcare regulations.
  • Implement and oversee compliance management practices to safeguard patient information.
  • Organize and classify metadata for efficient retrieval and storage of medical records.
  • Oversee archives management, ensuring proper archival processing of historical medical documents.
  • Utilize SharePoint for document collaboration and sharing within the healthcare team.
  • Conduct regular audits of medical records to ensure adherence to information security protocols.
  • Collaborate with healthcare professionals to streamline data management processes and improve record accessibility.
  • Prepare, scan and verify documents into the medical record daily.
  • Audit resident records at admission, quarterly, and at discharge for completeness, accuracy, consistency, and timeliness within the required timeframe. Ensures that records requiring signatures are received and filed within the required timeframe. Reports results, on a routine basis, of all audits to the Administrator, Director of Nursing, and MDS Coordinator.
  • Maintain resident medical information in chronological order, thinning or destroying records according to established procedures. Stores closed hard copy clinical records to ensure easy retrieval and sending records to offsite storage according to the requirements of retention as dictated by Corporate policy.
  • Maintains and controls disclosure of patient-identifiable information to authorized sources (persons) as outlined in HIC Quick Reference Job Responsibilities (MPRO, Attorneys, Insurance Companies, Social Services, etc.)
  • Requests information from other agencies / parties concerning Facility’s Residents for medical record completeness, and as directed from Interdisciplinary Team.
  • Provides information to those involved in QAPI
  • Adhere to patient rights and maintain confidentiality in accordance with HIPPA guidelines.
  • Attend staff meetings and provide in-services as requested.

Skills

  • High school graduate or equivalent required.
  • Proficiency in iManage for document management and workflow automation.
  • Strong understanding of compliance management within the healthcare sector.
  • Experience with metadata management to enhance record organization.
  • Knowledge of archives management principles and archival processing techniques.
  • Familiarity with SharePoint for effective collaboration on medical documentation.
  • Expertise in information governance practices to ensure data privacy and security.
  • Solid grasp of information security measures relevant to medical records handling.
  • Competence in data management strategies to optimize record keeping efficiency.
  • Must pay extreme attention to detail.
  • Excellent written, verbal, and interpersonal communication skills.
