Recovery Care Services seeks to strengthen and support individuals in recovery to live their best life. We achieve this by providing person-centered and recovery-oriented care coordination services that empower individuals through connections with a range of supports and treatments that optimize overall health and well-being.

 

Position Description

The State Care Coordinator provides person-centered and recovery-oriented care coordination services that assist individuals who are transitioning from publicly funded and fee-for-service substance use treatment to the next level of care by connecting them with services that support their continuing recovery. This is accomplished through a compassionate and collaborative working alliance with individuals served for up to 6 months post-transition from a substance use treatment facility.

 

Position Responsibilities:

  1. Demonstrate effective use of person-centered, recovery-oriented, motivational skills to engage clients in recovery care services and treatment programs.
  2. Conduct person-centered, trauma-informed, intake assessments prior to the conclusion of an individual’s residential treatment, using a collaborative and strengths-based interview process.
  3. In collaboration with the person served, create an individualized, strengths-based Service Plan.
  4. Execute Service Plan including referrals and connections with all services and programs outlined utilizing professional customer service-oriented referral interactions with internal and external agencies and individuals.
  5. Update the Service Plan to reflect accomplished goals and objectives with every interaction with the client.
 
  1. Conduct a client satisfaction survey at month 2, month 4, and upon discharge.
  2. Connect and document all contacts with individuals, making two live connections per month at minimum (phone, in-person, virtual) for up to 6 months of services. Exceptional care coordination will provide connections to program participants more than twice per month.
  3. Coordinate and provide ongoing assistance accessing and engaging in services as outlined in the Service Plan.
  4. Participate in follow-up meetings at the treatment facilities, residence of person served, HCAM offices, or other facilities as identified by individual or care coordination team.
  5. Utilize electronic records to capture all care coordination activities.
  • Ensure all client documentation adheres to the program standard operating procedures, program requirements, and/or HCAM guidelines
  • Place all client contact, encounters, and activities into designated the electronic health systems’ field to ensure accurate data collection.
  1. Consistently achieve productivity goals and performance measures established for state care coordinators.
  2. Build customer-oriented, collaborative relationships with treatment facilities and providers.
  3. Enroll uninsured clients into available health insurance programs; assist them in choosing an MCO and identifying a primary medical provider.
  4. Perform other duties and assignments as assigned.

Qualifications


Qualifications:

  • Experience providing case management services
  • Experience working with clients with substance use challenges
  • Active Driver’s License in good standing.
  • Bachelor’s degree in a human service field

Compensation

In compliance with Maryland's wage transparency requirements, the compensation for this position includes a base salary range of $ $43,500 per year. Additionally, the benefits package includes vacation, sick, personal, and mental health leave, a 403(b) retirement plan with employer match, medical, dental, life, short-term and long-term disability insurance, company-paid life insurance, voluntary life insurance options, Health Savings Account (HSA) and Flexible Spending Account (FSA) options, and a voluntary emergency savings fund with employer match.

  • Sensitivity to cultural and socioeconomic characteristics of the population served.
  • Excellent oral and written interpersonal and communication skills required

 

Physical Requirements

While performing the duties of this job, the employee is regularly required to talk, hear and respond professionally and courteously to internal and external customers and employees. The employee frequently is required to stand; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee is occasionally required to walk and sit. The employee must regularly lift and/or move up to 25 pounds and

 

occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The employee must be able to safely operate their own vehicle to provide onsite community services. 

*Float State Care Coordinator(s) must be able to provide on-site community services in multi-jurisdictions.

Work Environment

While performing the duties of this job, the employee is exposed to both indoor and outdoor conditions and may be subject to corresponding noise levels.
