General Purpose of Position :
The Licensed Vocational Nurse (LVN) is responsible for providing skilled nursing to the children in the PPECC setting. All care is provided in accordance with the Physician’s Plan of Care (POC) and within the state specific LVN Practice Act.
Primary Responsibilities :
- An LVN providing services on behalf of a center must work under the supervision of an RN and is responsible to provide, within the LVN's level of competence and scope of practice, nursing care to the center's minors as ordered in the plan of care.
- Maintains compliance with the standards of nursing practice.
- Providing nursing interventions that include parental training, information, and education to increase a parent's confidence and competence in caring for a minor.
- Participating in the IDT and in the IDT meetings regarding a minor's plan of care and progress.
- Administering medication, parenteral feeding, and other specialized treatments; monitoring and documenting the effect of medications, therapies, and progress in accordance with accepted standards of professional practice.
- Delivers nursing care in compliance with the individual Plan of Care and ongoing physician orders.
- On‐going observation and assessment of children / individual 's condition and needs.
- Takes appropriate action based on a child / individual's assessment.
- Documents all care delivered and the child’s response in the EMR.
- Identifies in writing a child / individual's responses to treatment, specifically those that are unusual or unexpected.
- Records, monitors, documents, and interpret vital signs.
- Provides personal hygiene assistance.
- Monitors child / individual’s intake & output.
- Maintains adequate medical supplies.
- Assists with the development of a child / individual's plan of care.
- Outlines child / individual goals and desired outcomes.
- Collaborates in the identification of child / individual needs and / or problems.
- Reinforces child / individual's and family education for care of the child / individual.
- Intervenes as appropriate to prevent or reduce a child / individual's injury.
- Maintains privacy and supports the rights of children / individuals and their families related to child / individual's care.
- Communicates effectively with the family and company staff to provide appropriate information relative to medical care.
- Works in a collaborative manner with other health care personnel.
- Acts in accordance with the Nurse Practice Act while complying with the Company’s Policies and Procedures, mission, and values.
- Reports any change in child’s condition, accident, or incident to the Registered Nurse.
- Ensures the safe transport and delivery of children when assigned to transportation as the medical escort.
- Properly secures children in car seats or wheelchairs for transport.
- Maintains appropriate professional credentials and accepts responsibility for personal and professional development.
- Completes all Company assigned education upon hire, monthly and annually per Company policy.
- Must maintain required and assigned continuing education.
- Performs other duties as assigned.
Education, Skills, and Experience Requirements :
one year of pediatric specialty experience with emphasis on medically and technologically dependent minors obtained within the last consecutive five years; or
