What are the responsibilities and job description for the Supervisor, Care Management (San Fernando Valley, CA) position at Healthcare in Action?
At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.
Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.
The Supervisor, Care Management will enhance patients experiencing homelessness’s ability to manage their health and live independently by conducting care management activities, including assessment, planning and implementing interventions that support success, with an emphasis on housing navigation in the San Fernando Valley, CA area. Supervises and leads non-clinical Healthcare in Action team responsible for coordinating patient care accordingly.
Responsibilities include:
- Develop individualized care plan utilizing evidence-based guidelines and clinical knowledge for clients by evaluating and conducting onsite visits, gathering assessment information, and identifying problems, goals and interventions.
- Deliver ongoing care coordination/care management by coaching, educating, navigating and referring to appropriate care and community-based resources.
- Assist patients in taking an active role in managing health and social needs by coaching, using motivational interviewing, and educating about self-management tools and strategies. Coach and support Care Management team to do the same.
- Develop cross-functional relationships with housing and allied community agencies by attending appropriate meetings, working on-site, and structuring effective communication mechanisms.
- Leads Care Management team efforts to coordinate with patients' various care team to identify barriers to care plan by sharing assessment information.
- Complete timely and accurate documentation in multiple computer systems to record assessment and corresponding documentation, including care plans and progress notes.
- Exhibit strong interpersonal, critical thinking and analytical skills through positive communication with clients, caregivers, care teams, and community agencies.
- Demonstrate excellent organizational, decision-making and multi-tasking skills as demonstrated by problem solving and successful outcomes.
- Enhance skills and knowledge by participating in team case conferences and training, per department guidelines.
- Utilize department desktop procedures, workflows, job aids and training materials. Identify barriers to work processes and brings to the attention of the supervisor/manager.

Maintain the client ‘s right to privacy and protect HIA operations by keeping information confidential and ensures Case Management does the same.

Adhere to all quality, compliance and regulatory standards to achieve outcomes.

Maintain professional and technical knowledge by attending educational workshops; reviewing and contributing to professional publications; establishing personal networks; and participating in professional societies.

Ensures team is driving efforts by accomplishing results according to role scope.
- Supervise/Manage 2 team members (i.e., hires, performance reviews, corrective action, etc.)

Qualifications and experience:

Bachelor’s Degree in Social Work, Gerontology or related field of study preferred, but not required.

Master’s Degree in Social Work, Gerontology or related field of study, preferred.

3 years people leadership with some direct supervision experience

3-4 years of experience in homeless care management field with a bachelor’s degree, or
-
2 years of experience with a master’s degree.

Work with street outreach is a plus.

Bilingual in English/Spanish preferred.

Work is primarily in the field, must be willing to travel in San Fernando Valley. Bi-monthly office visits to Long Beach.

Demonstrated knowledge of assessment, health and functional problems of homeless older adults.

Knowledge of community resources for seniors.

Basic knowledge of related CMS and DHCS regulations.

Strong interpersonal skills, including excellent written and verbal communication skills.

Strong organizational skills: Ability to multitask.

Ability to appropriately maintain confidentiality.

Strong analytical and critical thinking skills, required.

Requires proof of Valid Driver’s License
