Your Job
- Manage and direct the Social Services Department, and identify and provide for each residents social, emotional, and psychological needs
- Manage department to assure all psychological and psychosocial needs of residents are met
- Understand the MDS process with ability to interview, input, and complete social service assessments
- Accurately assess, document and provide interventions to meet psychosocial needs of all residents; develop care plans based on assessments
- Provide effective facilitation of resident care conferences: schedule, send letters to families/ responsible parties, and participate in care conferences
- Facilitate all resident discharges (arrange for home health, DME, assist with transfer to other facilities)
Who You Are
- Holds a bachelor's degree in social work or a closely related field (Have other qualifying skills? We want to hear from you!)
- Motivated and organized team-player
- Experience with social work, preferably working directly with patients and their family members at a skilled nursing facility
- Detail oriented, computer literate and have excellent communication skills
