What are the responsibilities and job description for the Director of Social Work position at Healthcare Nursing Center?

Essex County, NJ area Skilled Nursing Facility is in search of a Director of Social Work.
Qualifications:
- Bachelor's degree from accredited school of Social Work or related field and State of NJ Social Work License required
- Must be at minimum, an active, certified social worker (CSW)
- Must possess required certifications/licensures as required by the State of New Jersey to practice in a long-term care facility.
Specific Functions:
- Orients resident and family to facility, helping both adjust to new environment
- Develops a social history, assessment and care plan on each resident
- Completed the Social Services section for the MDS
- Ability to listen and act compassionately with residents’/family members; must be able to work on own and maintain related jobs
- Prepares and completes social service documentation in timely manner
- Assures that notes are informative and descriptive of services provided and of residents' responses to services
- Participate in resident care plan conference and is active member of care plan team
- Refer resident/family member to appropriate social service agencies when facility does not provide services or needs of resident
- Document residents' behavioral and psychosocial status
- Serves to reinforce to residents and families the facility's desire to deliver highest quality of care
Administrative Functions:
- Plan, develop, organize, implement, evaluate, and direct the facility's social services department. Develop, administer, and coordinate department policies and procedures
- Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to Administrator
- Participate in community planning related to interests of facility and services and needs of resident and family
- Perform administrative requirements, such as completing necessary forms, reports etc. and submitting such as required
- Make written and oral reports/recommendations to Administrator and relevant department heads concerning operation of social service department
- Be prepared to handle emergencies as they arise (i.e. rescheduling work assignments and work)
