Director of Social Work

Location: In-Person | Schedule: Monday–Friday | Day Shift

Position Overview:
The Social Worker plays a key role in supporting residents and families through various phases of care. This position involves collaboration with interdisciplinary teams, discharge planning, and facilitating emotional and social support services.
Key Responsibilities:
  • Advocate for residents’ emotional, social, and medical needs
  • Provide counseling and crisis support to residents and families
  • Participate in care planning and progress meetings
  • Support new residents during admission and transition
  • Coordinate discharges and ensure seamless transitions of care
  • Maintain accurate documentation in line with regulatory standards
  • Participate in quality improvement and compliance activities
  • Ensure social services are delivered in alignment with best practices and policies

Qualifications:
  • Bachelor’s degree in Social Work or a related human services field required
  • Master of Social Work (MSW) preferred
  • At least 2 years of social work experience, including 1 year in a healthcare setting
  • NYS license as an LMSW or LCSW required
  • Experience working in long-term care strongly preferred
