Job Description

We are seeking a skilled Physiatrist/Physical Medicine & Rehab Physician to join our team in Northern NJ. As a member of our team, you will have the opportunity to work in a SNF setting, providing consultation and follow-up care to patients. You will work closely with our patients, their families, rehab staff, physicians, and hospital system pop-health teams to deliver comprehensive care.

Responsibilities

• Provide expert physiatric consultations and follow-up care to patients throughout the rehabilitation stay.
• Collaborate with patients, families, and healthcare professionals to develop and implement individualized care plans.
• Conduct rounds on post-acute care patients as needed, tracking patient progress and removing obstacles to functional recovery.
• Maintain accurate records of patient care, following established protocols and guidelines for documentation, billing, and coding.
• Participate in facility rehab team meetings, contributing to the development and implementation of interdisciplinary care plans.
• Develop and maintain strong relationships with facility leadership, clinicians, and other stakeholders to ensure seamless care delivery.

Requirements

• Demonstrated expertise in physiatry and rehabilitation medicine.
