RN Director of Case Management
Greater Henderson Area, Nevada
The Director of Case Management leads, manages, and supervises all activities of the Case Management Manager, Team Leads, Case Managers, Social Workers and Case Manager Assistants to ensure appropriate Care Coordination and Care Transition handoffs for all patients.
Qualifications :
Registered Nurse required
Bachelor Nursing Degree required
Master's Degree preferred
Director of Case Management experience in an Acute Hospital Setting
Responsibilities :
Provides monthly metric reports and mitigation
Ensures effectiveness of the coordination and control of resources
Provides consultation and education to facility leadership and clinical personnel
