We are looking for a Social Service Coordinator at our skilled nursing facility.

Position Title: Social Services
Knowledge, Skills and Abilities: Requires demonstrated professional knowledge and skills necessary to plan, organize, and develop support services for our current residents and their families; the ability to establish and maintain effective professional working relationships with residents, families, and staff; knowledge of the inquiry and admissions process; effective verbal and written English communication; lift and carry up to 30 pounds.
Education: Graduate of an accredited institution with a bachelor’s degree in social work or extensive equivalent experience in the field.
Experience: Prior social work experience in the geriatric field preferred.
Certificate/Licenses: Bachelor’s Degree in Social Work field under the regulation with proper experience, valid Pennsylvania driver’s license.
