The Nurse Practitioner - Primary Care serves as a key contributor within our esteemed healthcare team, delivering comprehensive clinical services in a collaborative environment. We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care.
At the direction of the Medical Director, the Primary Care Provider works collaboratively with this medical provider and the IDT, providing assessments, planning and primary care, and continuing medical education to their patients.
Responsibilities
Deliver comprehensive geriatric primary care services including history taking, physical examinations, diagnosis, and treatment planning.
Collaborate closely with interdisciplinary teams to ensure the formulation of integrated, patient-centered care strategies.
Order, interpret, and manage diagnostic tests and laboratory investigations to inform clinical decision-making.
Educate patients and families on disease prevention, health maintenance, and chronic condition management.
Maintain meticulous medical records and documentation in accordance with regulatory and institutional standards.
Uphold professional practice guidelines and ethical standards to ensure quality patient care.
Engage in continuous professional development and quality improvement initiatives to advance clinical practice.
Essential job summary
Perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participants
Collaborate with physicians and IDT to develop care plans for each participant
Collaborate with physicians and IDT in discharge planning from acute or long-term settings
Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
Work collaboratively with WelbeHealth physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination
At the direction of the Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements
Take part in rotating night and weekend call
Qualifications
Holds a Master’s degree in Nursing with an accredited Nurse Practitioner specialization in Primary Care.
Possesses national certification in primary care, ensuring compliance with stringent practice standards.
Demonstrates competence in performing comprehensive health assessments, diagnoses, and treatment plans.
Utilizes evidence-based guidelines to deliver high-quality care in the prevention and management of chronic conditions.
Maintains advanced clinical skills for the management of complex patient cases in diverse primary care settings.
Engages in continuous professional development to stay abreast of the latest healthcare innovations and practices.
Upholds a commitment to ethical practices and interdisciplinary collaboration to optimize patient outcomes.
Current knowledge of chronic care / geriatric issues and best practices

Salary Range : $181,749 - $254,499
Additional incentives : up to $25,445.Competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
