Job Description

Healthcare Staffing Professionals has an immediate need for a Medical Case Worker to join the Youth Suicide Prevention Pilot team at the Los Angeles County Office of Violence Prevention in Los Angeles, CA.

The Medical Case Worker will (under professional direction) function as a member of a multidisciplinary team, providing discharge and referral case management services to families of youth patients (ages 25 and under) who have experienced a suicide loss or are being discharged after an Emergency Department visit related to active suicidality. This role will also provide support as needed to families of youth who have died by suicide. This role may continue to provide services to clients and families after discharge to ensure linkages to services necessary for ongoing support for the family after the youths suicide attempt or death.

The positions will begin immediately upon acceptance and employment clearance and are initially funded through 6 / 30 / 25.

Pay Rate :

29.65 / hr (Mileage reimbursement available)

Shifts :

40-hour work week
Hybrid : Office Work in DPH Site in Alhambra, California.
Field Work at LA General Medical Center, Olive View Medical Center or Harbor UCLA Medical Center.
Limited virtual “telework” may be possible.

Job Duties :

Case management will be provided to patients and clients within three Los Angeles County Department of Health Services Hospitals and may continue after discharge to ensure linkages to services necessary for ongoing support for the family after the youths suicide attempt or death.

Provide case management case management and linkages to resources.

Coordinate with care team to ensure proper timing and level of case management need.

Provide psychoeducation or other forms of health education to youth and / or families.

Documents the service provided in the electronic health record

Work with psychiatric social workers when needed, if patient follow up if limitations are present (e.g. transportation, etc,)

Skills / Experience :

At least one year of experience providing case management services to patients and clients in hospitals, clinics, rehabilitation centers, residential, or other health care facilities.

Must demonstrate prior experience working within youth populations.

Experience in mental and behavioral health

Experience providing psychoeducation or other forms of health education to youth and / or families

Ability to support youth at risk of suicide / suicidal behavior and their families.

Previous experience providing case management and linkages to resources.

Be able to function efficiently and positively in stressful situations.

Excellent communication skills.

A valid California Class C Driver License or the ability to utilize an alternative method of transportation to perform essential job-related functions

Required : Bilingual (Spanish)
