Care Transitions Manager - RN
HealthCare Support is actively seeking a Care Transitions Manager RN to fill an opening with a leading Healthcare organization in The Bronx.
Daily Responsibilities for the Care Transitions Manager :
- Delegate responsibilities and supervise personnel in collaboration with the Care Transitions Manager to ensure quality care and compliance. Evaluate participants' health needs considering physical, mental, and environmental factors, and guide home health staff in delivering services. Collaborate with the interdisciplinary team to develop and maintain comprehensive care plans and ensure all documentation and regulations are met while maintaining positive relationships and staying current with health practices
- Shift
- Monday - Friday 8:30 am - 5:30 pm | No on Call
Required Qualifications for the Care Transitions Manager :
- Clear and active NY State RN License
- 5 years as an RN
- 1 years of recent supervisor experience
- 1 years of Home Health

- $135K-$140K | 10K Sign on Bonus!!
