Care Transitions Nurse Manager - RN
HealthCare Support is actively seeking a Care Transitions Nurse Manager - RN to fill an opening with a prestigious Healthcare company in Long Island, NY.
Daily Responsibilities for Care Transitions Nurse Manager - RN :
- Will be making field visits but would not have their own caseload.
- Assisting and overseeing some of the more complex cases - act as a SME for Community Health Nurses in the field
- Communicate with interdisciplinary teams, social workers, and dietitians, helping coordinate that care
- Monday – Friday 8:30 am – 5:30 pm I On-call required
- This is a hybrid position with onsite & in-field component
Required Qualifications for Care Transitions Nurse Manager - RN :
- Active NY Registered Nursing license
- 3 years' experience as a Registered Nurse / RN
- 2 years of recent experience at a Certified home health agency (CHHA)
- 1 year of recent supervisory experience

Pay Details: $135,000 - $140,000 annually
- $10,000 Sign on Bonus
