In-Home Care Provider / Physician
HealthCare Support seeks an In-Home Care Provider / Physician to fill an opening with a Medicare Advantage Health Plan partner expanding into the Georgia market!
Daily Responsibilities for In-Home Care Provider / Physician :
- Determine the need for, and facilitate care by, an emergency department, hospital, skilled nursing facility, home health agency, hospice and/or community-based organization as appropriate and consistent with patients’ goals of care
- Meet practice goals, like helping patients stay at home whenever possible
- Engage in interdisciplinary team meetings with a focus on collaboration, learning, and teaching
- patient care, review outcomes and results, and update policies and procedures
- Caseload: See 6-7 patients daily and spend 45 minutes with them. This allows me to learn more about them, their families, and their home conditions.
- Oversee 7 to 11 In-home NPs. Support and guide nurse practitioners to ensure quality care, strong performance, and professional growth
- Hybrid: Monday- Friday, 8:00 AM – 5:00 PM.
- 4 days in field/telehealth, 1 day is administrative
Required Qualifications for In-Home Care Provider / Physician :
- MD/DO degree and are Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatric Medicine and/or Palliative and Hospice Medicine (required)
- Active license to practice in the state of Georgia
- Active CDS and DEA
- Experience with house calls setting previously, fee-for-service and/or value-based
- You enjoy working with patients of diverse social, ethnic, and cultural backgrounds
- Fellowship in geriatrics preferred

Pay Details:
- $220,000 - $260,000
- Up to 15% bonus equity
