HealthCare Support is actively seeking a Field Nurse Practitioner / Physician Assistant to fill an opening with a growing Healthcare provider in Jupiter, FL
 
Daily Responsibilities for Field Nurse Practitioner / Physician Assistant:

• The Provider works in a field-based capacity completing visits to patient homes for Chronic Kidney Disease / Primary Care needs. Providers also have telehealth opportunity based on patient need and location
• Job duties include complete history and physical, creation of treatment plan, order of diagnostics and lab testing, prescribing of medications, routine team meetings, patient education, care plan assessments, documentation of services
• Schedule: Monday-Friday, 8am-5pm

Required Qualifications for Field Nurse Practitioner / Physician Assistant:

• Licensed NP or PA in Florida
• 5 years of experience as an NPPA
• Completion of clear Background check and drug testing

Pay Details: $120,000 - $145,000 per year 10% Annual bonus
