Community Health Worker
HealthCare Support is actively seeking a Community Health Worker to fill an opening with a Managed Care Organization in Atlanta, GA .
Daily Responsibilities for Community Health Worker :
- Responsible for conducting home visits to high-risk members to complete a non-medical assessment to identify barriers impeding care such as home safety issues, transportation and other social determinants of health.
- 80% of job is in the field
- Connecting with community partners and / or organizations to build partnerships that help to address the needs of our members.
- Coordinate, implement and participate in community events within the designated region but could include the entire Georgia market.
- Shift : Monday-Thursday, 9am-6pm and Friday, 8 : 30am - 5 : 30pm (may have some OT and weekends, need to be flexible)
- Hybrid position
