HealthCare Support is actively seeking a Hospice RN Case Manager / Registered Nurse to fill an opening with a prestigious Hospice organization in Tennessee. Location - Hendersonville area
Daily Responsibilities for Hospice RN Case Manager / Registered Nurse:
- This RN will manage an assigned caseload of in-home hospice patients and provide ongoing end of life care including medications, monitoring, vitals, and emotional support
- The RN Case manager will work alongside an NP, RN team, Social worker, and other case managers to collaborate and promote ongoing care
- Mon-Fri, 8am-5pm
- On-call one night per week from 5pm-11pm (no weekends)
- Active RN License in TN
- At least 2 years of RN experience
- Related experience in – Hospice, Home Health, Oncology, ICU, med/surg, LTC/SNF
- Mileage reimbursement
- Full health benefits
- PTO - 6.77 hours accrued per pay period (increased after 3 years of employment) – starts accruing day 1
- Extended Sick Bank - 2.77 hours per pay period - starts accruing day 1
- 403b - company matches up to 3% of employee contribution at 50% rate
- Annual merit increases - up to 3% based on performance
- No weekend on-call!
