LCSW / LMFT Behavioral Health Case Manager

HealthCare Support is actively seeking a LCSW / LMFT Behavioral Health Case Manager to fill an opening with a Managed Care Company in Rancho Cucamonga, CA. This is a Hybrid role which requires 3 days in office per week.

6 month contract - possibility to convert/apply to internal positions once contract is complete

Daily Responsibilities for LCSW / LMFT Behavioral Health Case Manager:

• Under the direction of the Behavioral Health and Care Management Leadership, this position provides high quality, effective care management to members.
• Outreach and engagement to members, engaging members in skilled therapeutic interactions to promote health behaviors, coordination of care, resource linkages, working with other professionals and organizations in the community to ensure quality of care for members, seamless transitions of care, and facilitating the right care and the right time for the member.
• This position works collaboratively with members of their own team, members and families, and other professionals.
• As a licensed clinician, this position provides clinical expertise, clinical leadership, and clinical oversight in a variety of ways within the Behavioral Health and Care management departments.
  
  
  

Required Qualifications for LCSW / LMFT Behavioral Health Case Manager:

• Master's degree required
• Active, unrestricted, and unencumbered LCSW or LMFT
• 3 years of behavioral health experience
• Familiar with managed care and discharge planning
• Soft skills: strong communication skills, team player, ability to engage members and providers on the phone
• Computer Skills: Excel, Word, Outlook
  
  
  

Preferred Qualifications for LCSW / LMFT Behavioral Health Case Manager:

• MCO experience
• Case Management experience
• Bilingual in Spanish

Pay Details: $43.87 an hour
