HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with one of the nation’s largest healthcare organizations here in Somerset County, NJ.
Daily Responsibilities for the Nurse Practitioner:
- Deliver preventive care to members in the comfort of their home. (Company Vehicle Provided, Medical Assistant will drive the Vehicle)
- Complete a comprehensive review and assessment of a member's medical history, physical health, environment and psycho-social status.
- Document progress notes, prepare member workups and encounter summaries.
- Help at-risk members transition from the hospital and other facilities to their own home.
- Create personalized care plans in coordination with the member's primary care physician.
- Shift: Monday - Friday, 8:00 a.m. - 5:00 p.m.
- Must be based in the state of New Jersey
- Certified as an Advanced Nurse Practitioner in specialty areas
- Hold current unencumbered NJ Licensure as a Registered Nurse
- Active DEA registration
- Minimum of two (2) years’ experience as an Advanced Registered Nurse Practitioner
- Master’s degree in advanced clinical practice nursing or graduate of post Master’s certificate program for Nurse Practitioner
- Current provider card in Basic Life Support (BLS)
- Willingness to travel to the member’s home and therefore must have reliable transportation
- Experience working with geriatric patients
- Home care experience
- Medical, Dental and Vision
