Key Responsibilities:
- Provide direct patient care in a home health setting, working closely with patients, families, and interdisciplinary teams to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing targeted interventions.
- Collaborate with healthcare professionals to coordinate care and ensure seamless transitions between settings.
- Document all patient interactions and outcomes, maintaining accurate and up-to-date records.
Requirements:
- Active RN license
- At least one year of experience in the specialty
- BLS Certification from the American Heart Association
