HealthCare Support is actively seeking a RN Case Manager to fill an opening with a large Healthcare organization based in Portland, OR

Daily Responsibilities for RN Case Manager:

• The Case Manager will coordinate care for health plan members, work with providers to ensure proper care, and educate on health maintenance and prevention

• Schedule: Monday-Friday, 8am-5pm

Required Qualifications for RN Case Manager:

• Active RN License in Oregon
• 3 years of clinical experience
• Case management experience

Pay Details: $75,000 - $90,000 (based on experience)
