Daily Responsibilities for the Remote RN Case Manager:
- Coordinate care for members across Commercial, Medicare, and Medicaid lines of business, focusing on one area at a time based on business needs.
- Conduct telephonic case management, assessing members’ health needs, creating care plans, and working with providers to ensure proper treatment.
- Advocate for patients, provide education on condition management, and monitor progress to improve health outcomes.
- Collaborate with doctors and healthcare professionals to deliver holistic, patient-centered care.
- Monday - Friday, 8 AM - 5 PM
- Tuesday - Saturday, 8 AM - 5 PM
- Sunday - Thursday, 8 AM - 5 PM
- Must reside in Oregon
- Active and unrestricted RN License in Oregon
- 3 years of RN experience
- Experience in one or more of the following areas is ideal: clinical assessments, referrals to medical resources, care planning and discharge planning
- Telephonic case management experience
- Certified Case Manager Certification (CCM)
