Our client is actively seeking an experienced Family or Adult Nurse Practitioner to support their busy post acute care facility. This is a flexible part time opportunity. 1 to 2 days per week you choose the schedule.
Key Responsibilities :
- Patient Care : Provide direct clinical care to residents including assessment diagnosis treatment and ongoing management of medical conditions.
- Comprehensive Assessments : Conduct physical exams review medical histories order and interpret diagnostic tests and develop treatment plans tailored to individual patient needs.
- Medication Management : Prescribe and manage medications including pain management antibiotics and other therapies as appropriate for postacute conditions.
- Care Coordination : Collaborate with attending physicians nurses physical therapists social workers and other healthcare providers to coordinate care plans and ensure effective transitions of care.
- Family Education and Support : Educate patients and families about treatment plans medications and lifestyle modifications to promote recovery and wellness.
- Clinical Documentation : Maintain accurate uptodate electronic medical records in compliance with facility and regulatory standards.
- Chronic Disease Management : Monitor and manage chronic conditions such as diabetes hypertension COPD heart disease and others commonly encountered in postacute care settings.
- Quality Improvement : Participate in quality improvement initiatives to enhance patient outcomes and ensure compliance with healthcare standards and best practices.
- OnCall Responsibilities : Includes some afterhours or weekend coverage to address urgent medical issues.
