Schedule : Full-Time
Pay : $68.80 - $79.35 per hour
Requirements : Fluent in Mandarin or Cantonese Required

Position Summary :
The PACE Clinic RN works under the supervision of the PACE Center Manager , providing direct and indirect nursing care to PACE Participants . This role involves collaboration with physicians, clinic staff, and the Interdisciplinary Team (IDT) to manage patient care in a primary care setting within the PACE Center. The RN ensures quality care, patient education, and smooth care transitions.
Key Responsibilities :
- Conduct patient assessments to determine care needs and goals.
- Participate in IDT meetings for care planning.
- Administer medications, treatments, and tests.
- Monitor and update care plans based on patient conditions.
- Coordinate patient transitions between care settings.
- Educate patients, families, and caregivers on health management.
- Assist with new enrollments and end-of-life care.
- Respond to medical emergencies within RN scope.
- Accurately document care in Epic EMR .
- Supervise and support LVNs and MAs for efficient care.
