Description
HTS is currently seeking a dedicated Physical Therapist to join the therapy team on a PRN basis at our partner continuing care retirement facilities located in New Carlisle and South Bend, Indiana. This is an excellent opportunity for licensed new graduates as well as experienced professionals looking for flexible work schedules. PRN with HTS is extremely flexible however a mixture of weekend and weekday availability is requested.
Key Responsibilities:
- Conduct evaluations for residents to assess their physical therapy needs.
- Document progress notes, recertifications, and discharge summaries on a PRN basis.
- Collaborate with interdisciplinary teams to develop and implement individualized treatment plans.
Qualifications:
- Valid Physical Therapy license in the state of Indiana.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work according to staffing needs.

Requirements
Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn an Indiana License, including completion of a nationally accredited program and subsequent passing of national licensure exam.
Applicants must be legally eligible to work in the United States. Proof of eligibility will be required upon offer of employment.
