Job Description :

Providers treat injuries and medical illnesses that occur in everyday living. Broken bones, sinus infections, colds, and other common viruses are examples of ailments that are treated by a family practice provider. When patients have more serious conditions, they are typically referred to a specialist. The Provider interviews the patient and diagnoses the patient by determining the problem. Multiple tests, such as examining eyes, ears, or throat, can be done easily to narrow down the patient’s concerns. Other tests can be performed as needed. Communication with patient’s care team and documentation within the electronic medical records system are essential to job duties.

Patient-Centered Medical Home Essential Functions :

1. Patient Population Management :

a. Use electronic medical records and registry to proactively manage care for all patients, but especially patients with certain medical conditions (such as diabetes or asthma) or those needing services (such as immunizations or mammograms).

b. Make diagnoses and decisions about management and treatment of patient conditions. For example, review on a regular basis long-term cases that require ongoing medical attention.

2. Care Coordination :

a. Refer patients to and work closely with the Kansas Department for Children and Families Program Consultant, located on the HCC premises, who enrolls Kansans in Medicaid, Medicaid CHIP, and Project Access.

b. Administer or order diagnostic tests, such as x-ray, electrocardiogram, and lab tests as needed.

3. Self-management Support :

a. Provide self-management support to patients by providing self-management tools and encouraging patients to self-manage and promote healthy behavior changes. For example, educate patients on wellness, prevention, and early detection by providing materials and resources to the patients and families.

b. Educate client on his or her specific condition to raise awareness about expectations, medication instructions, possible side effects, daily living, and proper health maintenance. This position requires attendance at Patient-Centered Medical Home (PCMH) meetings.

c. Instruct and counsel patient about prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living and health maintenance.

4. Communication Skills :

a. Display the ability to communicate effectively with vulnerable populations, including those made vulnerable by their financial status, personality characteristics, place of residence, health status, and / or age. Understand health literacy levels or other approaches to addressing communication needs.

5. Quality Improvement :

a. Work closely with Medical Director and team members to ensure compliance with policies, procedures, bylaws, regulatory requirements, and best practice guidelines.

b. Identify the need for staff development and make recommendations.

c. Participate in quality improvement, management, continuing education, and other patient care programs established by HCC guidelines.

Additional Essential Functions include the following.

  • Examine patients to obtain information about their physical condition.
  • Interpret diagnostic test results for deviation from normal.
  • Obtain, compile and record patient medical data, including health history, encounter notes and result of examination.
  • Prescribe therapy or medication
  • Perform therapeutic procedures, such as injections, immunizations, suturing and wound care, and infection management.
  • Responsible for the day to day supervision of medical team.
  • Assigns work to designated staff based upon the needs of the clinic and patient volume.
  • Establish employee performance standards and conducts annual reviews.
  • Provide training to staff on Electronic Medical Records system and Program Services to include :
  • Support implementation and maintenance of EMR.
  • Develops, coordinates and supervises staff within the clinical area.
  • Ensures adequate staffing, appropriate training, exceptional customer service, and quality care.
  • Works with the Medical Director to schedule staff in order to appropriately cover the hours of operation
  • Assigns, directs, and reviews the work of the staff.
  • Plans training and development of staff to improve performance.
  • Ensures compliance with policies, procedures, bylaws, regulatory requirements, and best practice guidelines.

Additional Responsibilities : As assigned and / or required

Qualifications and Requirements :

  • Knowledge of the principles and practice of preventive medicine.
  • Knowledge of the state and federal laws pertaining to medicine and to community health centers.
  • Knowledge of the structure and function of community health centers.
  • Valid Kansas State Board of Healing Arts Medical License.
  • BLS (Basic Life Support) Certification
  • Federal DEA Certification
  • Board Certification in area of training or board eligible for a period not to exceed one year from employment
  • Have thorough knowledge of the principles and practices of medicine and allied services commensurate with his / her training
  • Demonstrate a special interest in the area of community medicine
  • Ability to speak enough Spanish to examine Spanish-speaking patients and converse on basic level strongly preferred
  • Ability to relate to culturally diverse patients and community
