FOOD PANTRY / COMMUNITY HEALTH WORKER

Job Purpose : The Community Health Worker working in the food pantry is responsible for food pantry operations and maintenance and providing care for our patients through healthy food access. They serve as a link between the patient and social service agencies.
Daily Tasks :
1. Patient Population Management :
a. Ability to use software to track inventory and patient pantry visits, as well as pull reports for the Kansas Food Bank
b. CHW should be able to use PantrySoft to complete new client form and registration questions and be able to track visits with the appropriate food items.
2. Care Coordination :
a. Assist patient through the pantry, letting them select items and add to their bags if they are able to, in order to give them a dignified shopping experience and letting the patient know how many of each item they can select based on the Food Pantry Family Guidelines.
b. Assist patient load their items into their vehicle using one of the carts in the food pantry when necessary.
3. Self-management Support :
a. Provide education about self-management tools patients can use and encourage patients to self-manage and promote healthy eating changes.
4. Communication Skills :
a. Monitor analytics and create reports detailing the successes and failures of pantry operations.
b. Call Food Is Medicine (FIM) patients that have not received their food boxes on a weekly basis and coordinate deliveries with Facilities manager, enter this information into FIM Dashboard.
5. Quality Improvement :
a. Responsible to work with Food Pantry Manager and other members of the Quality Committee Team to develop, maintain, and implement plans to achieve quality improvement goals.
Additional Essential Functions include the following.
- Complete Temperature Logs (A.M. and P.M. in the pantry, chest freezer and the kitchenette areas).
- Check emails and Outlook calendar for Kansas Food Bank order pickups, meetings etc.
- Wipe down tables, chairs and desk areas with disinfectant wipes.
- Keep documents in stock & organized in pantry as resources for patients such as but not limited to resource guides and food pantry list, any produce giveaways / events and / or food distribution forms and coloring pages and kids corner word searches.
- Stock shelves and organize back stock. Assess canned goods and other food on non-food items while stocking shelves and always checking for expiration dates.
- Sort non-food items with zip lock bags if they arrive in large packages to allow more patients to have those items (i.e. feminine products, Band-Aids, toothbrushes, etc).
- Clean out Refrigerator and Freezer between orders and if they are empty.
- Assemble Food is Medicine Boxes, Welcome Kits and After Hour Food Boxes.
- Call Food is Medicine patients about pick up or delivery of their food boxes.
- Enter in patient information when the iPads are unavailable or After-Hours Boxes are distributed the previous evening.
- Do monthly inventory count with Food Pantry Manager and adjust Food Bank Orders.
- Complete all required documentation and data tracking in a detailed and timely manner.
- Attend and actively participate in all project related trainings and project meetings.
