Job Summary :
- Assist in coordinating the transfer of patients from home health services to Palliative or Hospice, resulting in uninterrupted continued medical care.
- Assist in coordinating the transfer of patients residing in a SNF or discharging from a hospital setting to Palliative or Hospice services resulting in uninterrupted continued medical care.
- Provide patient care, to include using a variety of skills and necessary coordination of services.
- Complete appropriate documentation as required by policy and procedure and regulation.
- Assist Clinical Manager and / or Hospice Administrator in day-to-day branch office tasks to ensure patient care documentation is complete and submitted timely.
- Assist Clinical Manager and / or Hospice Administrator in management of patient care supplies.
- Assists in Medicare part D medications management.
- Assists the Hospice Registered Nurse with medication reconciliation and Pharmacist medication recommendations.
