Acute Hospital Care at Home (AHCaH) Advanced Practitioners (APs) operate similarly to inpatient hospitalists with a more virtual care approach. Cooperating with other team members through huddles and as needed to assess patient needs and identify appropriate patients for this model of care. Physician Assistant must live within reasonable traveling distance to Danville, PA.

Program Highlights :
The AHCaH program utilizes remote patient monitoring, advanced technology, and interdisciplinary collaboration to deliver exceptional care to patients who meet hospital admission criteria.
Target Population & Conditions Treated :
- Med / Surgical level of care
- Congestive Heart Failure (CHF) exacerbation
- COPD exacerbation
- Urinary Tract Infections (UTIs)
- Other appropriate conditions
Your Role as a Physician Assistant :
As the core provider on the AHCaH team, you’ll work closely with patients and collaborate with a dynamic interdisciplinary care team under physician guidance. Key responsibilities include :

Based at the Geisinger Health Plan Hughes Center in Danville, PA, with travel to Geisinger Medical Center, Geisinger Bloomsburg Hospital, and Geisinger Shamokin Community Hospital as needed.
