Position Summary : The Care Coordinator is responsible for managing the coordination of patient visits, ensuring that the scheduling system and electronic medical records (EMR) are maintained accurately and up-to-date. This role involves the careful assignment of patients to appropriate clinicians, taking into account factors such as language, culture, and specific patient requests. The coordinator communicates scheduling changes promptly between office and field staff, monitors system messages, and responds appropriately in a timely manner. The Staffing Coordinator plays a vital role in ensuring the delivery of high-quality care to patients and their families.Essential Duties and Responsibilities : Is responsible for the overall staffing coordination of patient visits including but not limited to, maintaining accurate and update to date patient schedules within EMR and scheduling systems.Staffing coordination duties also include the accurate and timely communication of scheduling changes between office and field staff.Monitors system messages from office and field staff and responds appropriately in a timely manner.Match patients with appropriate clinicians. Take into account language, culture, patient request, and the clinician that would most benefit the patient.Maintain a current patient roster with necessary information.Assist in the coordination and facilitation of vendor services provided to patients as needed.Escalates scheduling conflicts, in ability to staff, etc. to the Manager or Clinical Supervisor as appropriate.Check compliance of visits done (on master schedule) after all notes are matched to charges, brings any scheduling problems to the supervisor's attention immediately.Assist Case Managers with assigning LVN's to follow up on patient careEnsures that appropriate supervisory visits are assigned and plotted within Medicare guidelines, e.g. LVN supervisory visit within every 30 calendar days and CHHA supervisory visit within every 14 calendar days.Obtain weekly visit count and report results to Clinical Leadership to assess utilization and productivity.Assist in relaying messages to field staff, office staff and community liaisons, gives and takes reports from the on call coordinator.Provides intermittent or on-going staffing coverage for any member or territory of the Care Coordination team as assigned by direct supervisor.Routinely runs and analyzes the and productivity reports and escalates to Manager and Clinical Leadership as appropriate.
