What are the responsibilities and job description for the Case Manager position at HealthHR, Inc.?
The Case Manager is responsible for arranging and monitoring schedules of therapists. It is important to be an organized self-starter who has the ability to juggle many tasks while also being able to differentiate between tasks that need immediate attention.
QUALIFICATIONS
- Experience in home care staffing/scheduling and Medicare is a requirement
- Strong computer skills required, preferably previous experience with staffing software
- Positive, cooperative, and supportive communication skills are required
RESPONSIBILITIES
The Case Manager will work directly with other Case Managers and agenices and collaborate with the Operations Manager to assure stability, continuation, and satisfaction on each case.
- Matches appropriate therapists to clients according to availability.
- Makes sure that all cases are covered and arranges for substitute/back-up coverage as necessary.
- communicates between therapists and agencies
- Handles/resolves caregiver issues and scheduling changes.
- Communicate and confirm schedules, including any changes.
- Maintains positive relationships with all clients, therapists and referral sources.
- Must have excellent problem-solving skills and be able to prioritize.
- Be detail-oriented with great time management skills.
