POSITION TITLE: Care Navigator
FLSA STATUS: Non-Exempt, Full-time
REPORTS TO: Program Manager
LOCATION: Applicants must be local to Santa Clara County and able to work in-office from our San Jose location.
Job Summary: The Care Advocate serves as a key liaison for individuals and families, connecting them with essential tools and resources. This role involves actively listening to clients' priorities and collaborating with various service organizations to provide comprehensive support, including basic health and wellness appointments, educational support, Medi-Cal application assistance, and coaching. Utilizing human-centered design approach, the Care Advocate helps clients set and achieve personal goals, leveraging deep understanding of community resources to identify and facilitate access to appropriate support.
Essential Functions & Position Responsibilities:
Enhanced Care Management (ECM)
- Act as liaison for individuals to access health care providers and community resources
- Schedule health and wellness appointments, preventative care, promoting the use of Primary Care Provider (PCP) and patient’s medical home
- Establish rapport with individual seeking services, practicing consistent follow-up, trust building and empathy skills by sharing responsive and accurate community resources.
- Complete client assessments to develop personalized care plans for members
- Assist health plan members in accessing preventative health services and provides intensive coordination of health and health related services for highest-need members. This includes working closely with healthcare staff, and managing client cases
- Advocate for plan members’ needs and collaborate with external organizations to address barriers to care

- Connect individuals/parents/caregivers with providing dental, vision, and basic needs resources
- Follow-up with individual referred through internal programs including vision, dental, hearing screenings and wellness checks, and educational programming.
- Identify uninsured children and assist them with enrolling into health coverage programs
- Collaborate with internal department and external partners to support HKF programming demonstrating the organization’s values of service, compassion, innovation, collaboration, accountability, and cultural sensitivity.
- Utilize Salesforce to document engagement with clients timely and accurately
- Collect health contact forms and manage well-being appointments for foster youth and other vulnerable populations
- Conduct service satisfaction surveys to curate family testimonials and capture continuous improvement opportunities
- Participate in continuous improvement discussions and processes
- Perform other duties as required by the needs of the organizations
- Participate in all required agency events
Minimum Requirements:
- Bachelor’s degree or equivalent experience required.
- CHW Certification required.
- 1-2 years of experience working with community-based organizations supporting marginalized and high-risk, underserved populations
- Must be fluent in Spanish, Vietnamese, or Mandarin
- Ability to work effectively under pressure
- Excellent judgment, time management, organizational, interpersonal communication, writing, and customer service skills
- Excellent computer skills (Microsoft Office Software including Word, PowerPoint and Excel) and database management skills
- Maintain confidentiality at all times
- Available 8-hour shifts; start times 8:30 – 10am, end times: 5 – 7pm
- Occasional Saturday or Sunday (as needed)
- Display flexibility in responding to changing circumstances and requirements
- Frequent travel within local community
- Ability to drive and have a reliable vehicle with car insurance
- May require travel within the community to meet with plan members or attend community events
- May be required to work evenings and weekends as needed

Frequent speaking, listening using a headset, use of hands/fingers across keyboard or mouse, long periods working at a computer.
