Description

As a Resident in our Behavioral Health Residency, you will provide brief behavioral health interventions in a primary care, law enforcement, or other behavioral health services. During your residency program, you will be expected to participate in different clinical rotations and/or shadowing experiences in-person, including but not limited to, federally qualified health centers (FQHC), law enforcement, clinical pharmacy, psychiatric inpatient and detox facilities, residential treatment, community mental health centers, and other behavioral health levels of care and services. You will also attend didactic trainings, conferences, and other trainings as assigned.  In this role you will be a part of the Integrated Medical Team and you will report to the local Site Operations Director with clinical and grant oversight from the Behavioral Health Integration and Residency Director.  

JOB RESPONSIBILITIES:

  • Maintains a presence and active communication to the Primary Care Providers (PCPs) during clinic operating hours.
  • Performs brief interventions and limited follow-up visits for selected patients. When necessary, refers patients to appropriate community mental health sites.
  • Maintains a presence and active communication to the providers and staff within the FQHC, law enforcement officers and administrators, or other clinical rotation.
  • Develop and maintain community relationships and participate in committees and meetings.
  • Available for same day and scheduled initial interventions with patients.
  • Performs brief, limited follow-up visits for selected patients. Whenever possible, refers patients to appropriate community mental health sites.
  • May provide occasional intensive case management and traditional medical social work services such as referral management and coordination, acquiring durable medical goods, and patient advocacy.
  • Provides a range of services including screening for common conditions, assessments, and interventions related to chronic disease management programs.
  • Conducts risk assessments, as indicated.
  • Provides psycho-education for patient’s families.
  • Assists in the development of clinical pathway programs, group medical appointments, classes, and behavior focused practice protocols.
  • Identifies, reviews, and modifies educational materials for literacy level and cultural appropriateness under the supervision of the Chief Behavioral Health and Addictions Officer.
  • Provides brief behavioral and cognitive behavioral interventions for patients and families
  • Triages patients with severe or high-risk behavioral problems to comprehensive behavioral health service (CBHS) or other community resources for specialty mental health (MH) services consistent with Step-up/Step-down criteria.
  • Provides PCPs with same-day verbal feedback on client encounters either in person or by phone.
  • Facilitates and oversees referrals to specialty MH/Substance Abuse (SA) services, and when appropriate, support a smooth transition from specialty MH/SA services to primary care.
  • Prepares brief consultant notes for the medical chart that explain assessment findings, interventions delivered, and recommendations made to the PCP.
  • Maintains clinical records and other necessary paperwork in a timely manner to comply with all administrative regulations.
  • Educates PCPs in the basic principles of brief behavioral interventions, evidence-based therapy and cognitive behavioral interventions and reinforce their use in the medical visit.
  • Provides assistance in capturing program evaluation and fidelity measures.
  • Will be expected to see between 10-12 patients per day, in-brief (15 minute) interventions. May also provide 30-minute therapy sessions.
  • Attends clinic meetings, including all staff, PCP, Clinic Leadership, and Clinic.
  • All HealthLinc staff is committed to engage in quality improvement initiatives that align with and support Patient-Centered Medical Home (PCMH).
  • Performs other duties as assigned.

Qualifications


REQUIRED QUALIFICATIONS:

Education/Training

  • Master’s degree in Social Work, Clinical Psychology, or Clinical Counseling
  • Must have associate license (LMHCa, LSW, or LMFTa) or temp license in Indiana prior to start date

  • Excellent written and verbal communication skills

Technology Skills

  • Operate a multi-line phone system and other office equipment including printers, fax machines, etc.
  • Basic computer skills (Microsoft Office, EHR, online sources, etc.)
